    The Latest in Hepatitis C Treatments

    Hepatitis C is the No. 1 cause of liver cancer and liver transplants. It’s brought on by a virus that you can catch if you come into contact with contaminated blood. You could get it from an unclean tattoo needle, for example. Sometimes, it spreads during sex.

    It can be cured. But doing so hasn’t always been easy or comfortable. For decades, you needed painful shots of a medicine called interferon and a pill called ribavirin. These drugs didn’t target the virus making you sick. Instead, they amped up your immune system, so you’d fight it like you do when you get sick with the flu.

    But the treatment didn’t always remove the virus from the body. Cure rates hovered around 50%. And, people who stuck with the year-long treatment — not all did — had to live with chemo-like side effects.

    These days there’s a revolution underway in hepatitis C virus (HCV) treatment. More and more people are being quickly cured by simply taking a pill, at home, for just a few weeks. Several injection-free options have been approved since 2014, and that list is growing fast.

    Here’s a closer look at some of the breakthrough drugs and a peek at those on the horizon.

    Ways the New Treatments Work

    There’s no one-size-fits-all option. There are many different types, or “genotypes,” of hepatitis C. Type 1 is the most common. This is important to understand when you talk to your doctor. Not all meds work on all types. Which medicine is best for you also depends on how much liver scarring (cirrhosis) you have.

    Your doctor might refer to the new drugs as direct-acting antivirals. They zoom in on the virus that’s making you sick. Each drug works in a slightly different way. But, in general, the medicine interferes with proteins in your body that the virus needs to grow or spread.

    Most of the time, the new medicines remove all traces of the virus from your blood within 12 weeks. This is called “virus clearance,” and it’s what doctors look for to determine if you’re cured. How long you need treatment can vary. It may range from 8 to 24 weeks.

    Meet the New Meds

    Daclatasvir (Daklinza): Approval of this drug meant no more shots for the 1 in 10 people infected with HCV type 3. It’s a pill you take once a day along with sofosbuvir (Sovaldi). You might get a headache or feel a little tired. Tell your doctor if you feel super-sluggish. The FDA warns it can sometimes cause a serious slowing of the heart rate, which may require you to get a pacemaker.

    Elbasvir and grazoprevir (Zepatier): This once-a-day pill treats HCV types 1, 4, 6. It may also offer new hope for people with hep C who also have cirrhosis, HIV, late-stage kidney disease, and other hard-to-treat health conditions. Like the other antivirals, the side effects are mild. You might have a slight headache, bellyache, or feel tired.

    Ledipasvir and sofosbuvir (Harvoni)

    This once-a-day pill launched the revolution in hep C treatment. It was the first interferon-free med for people with type 1. A year later, the FDA also gave the thumbs up for people with HCV types 4, 5, and 6 to use it. Side effects are mild. You might feel tired or have a slight headache. Some people have a belly ache, diarrhea, and trouble sleeping.

    Ombitasvir, paritaprevir, ritonavir (Technivie)

    This drug made a once-a-day pill option available to people with HCV type 4 who don’t have cirrhosis. You can take it with ribavirin, which may boost your chances of a cure. Common side effects are feeling tired or week, stomach upset, itching, and trouble sleeping. It might cause severe liver damage in people with advanced cirrhosis.

    Ombitasvir, paritaprevir and ritonavir, with dasabuvir (Viekira Pak)

    Doctors say this treatment works well for people with HCV type 1. You can even take it if you have some liver scarring, as long as your liver still can do its job. Your doctor might call this compensated cirrhosis. You take one pill once a day and another pill twice a day. Some people find this clunky, but others say it beats injections. Side effects include feeling itchy, weak, tired, or having trouble sleeping. This medicine might cause severe liver damage in people with advanced cirrhosis.

    Simeprevir (Olysio)andsofosbuvir (Sovaldi)

    The FDA said these two drugs could be given together to treat people with HCV type 1. Before that, the pills had to be taken with interferon or ribavirin. Sofosbuvir (Sovaldi) can cause fatigue, headache, tummy troubles, and make it hard for you to sleep. Simeprevir (Olysio) may cause dry skin and a rash, and make you more sensitive to sunlight.

    What’s On the Horizon?

    Researchers hope to soon have a single pill that can treat all types of hepatitis C. The combo drug sofosbuvir/velpatasvir looks promising. It clears the virus in 12-24 weeks, even in people with cirrhosis.

    Diet for Dogs with Canine Liver Disease

    Following a canine liver disease diet is an important treatment tool for dogs with compromised liver function. A dietary change for a dog with canine liver disease (CLD) helps the liver regenerate while also maintaining good canine nutrition.

    Liver Disease in Dogs

    Liver disease is common in dogs. It is especially prevalent in certain breeds such as West Highland Terriers and Doberman Pinschers. It is one of the top five causes of non-accidental canine deaths.

    As the cleaning system for the body, the liver removes toxins and waste. It also produces bile for the digestive process. When the liver is compromised, toxins and waste may build up in the body. This may affect many of the other bodily systems such as the brain and heart.

    The liver is remarkable in its ability to regenerate itself. With early detection and treatment, many CLD patients can recover completely.

    Canine Liver Disease Diet Development

    All significant diet changes should be thoroughly discussed with your veterinarian. CLD is not a condition that should be treated without medical guidance. Your vet will be able to structure a dietary plan that will help your pet recover.

    Most dietary treatments for a CLD patient include four basic goals:

    • Provide good nutrition to maintain energy and health
    • Promote liver regeneration and reduce stress on the organ
    • Prevent and minimize potential complications, such as hepatic encephalopathy, where the toxins affect the brain
    • Preclude and inhibit liver damage from the accumulation of substances such as copper.

    Protein Control

    Your vet will most likely recommend a change in the protein consumption of your dog. Liver disease usually means that less protein is being processed, so your dog’s protein intake will need to be monitored. The general recommendation is to ensure the protein consumed is high quality, but to keep the amount to a moderate level. Some of the protein may come from non-meat sources such as cottage cheese. High quality protein sources contain enough amino acids for your dog and are easily digested. Other recommendations may include offering plant-based proteins, such as soy, rather than meat-based proteins.

    In certain CLD complications, such as hepatic encephalopathy, the amount of protein may be reduced. Less protein will control the symptoms of that condition.

    Some animal proteins contain high levels of copper and should be avoided. Organ meat, especially liver, should be avoided. Other meats high in copper include:

    Fat and Carbohydrates

    With CLD, dogs are able to tolerate higher levels of fat in the diet. Your vet may recommend a diet that has up to 50 percent fat content.

    Carbohydrates are important to aid the digestion, add fiber and remove ammonia from the system. Cooked oatmeal, white rice and pasta are types of carbohydrates that may be included.

    Additives and Supplements

    Dogs with CLD, especially in the advance stages, should have a low-salt diet. Lowering salt prevents the build up of fluid in the abdomen, called ascites which occurs in dogs with low liver function.There are good supplements that may help your dog with CLD. Some of these supplements are:

    • Vitamin B complex
    • Vitamin E
    • Zinc, which helps bind copper and has antioxidants which protects the liver
    • Vitamin C, for antioxidant action
    • Vitamin K, for blood clotting
    • Adenosylmethionine (SAMe), which may reduce liver injury and also has antioxidant properties

    Commercial Diets

    Your vet may prescribe a special commercial dog food such as those made by Hill’s or Purina. These prescription foods are specially designed for dogs with liver problems.

    Feeding Routine

    Some dogs with CLD benefit from a change in feeding routine. Instead of one or two regular meals a day, several small meals throughout the day may promote good digestion.

    You should work with your vet to develop a canine liver disease diet for your pet. A good diet can help your dog feel better and heal faster.

    THURSDAY, April 14, 2016 (HealthDay News) — Here’s some potentially good news for people with hepatitis C who are waiting for liver transplants: Hepatitis C -infected livers seem to do as well as healthy livers in these patients, a new study indicates.

    The findings suggest that using hepatitis C -infected (HCV-positive) livers could help reduce wait times for people with hepatitis C who need a transplant, the researchers said. Hepatitis C is a virus that can infect the liver, leading to inflammation. scarring and liver cancer .

    More than 15,000 people in the United States are on the liver transplant waiting list, and about 16 percent will die before they receive a new liver, according to background notes with the study.

    In the United States, use of HCV-positive livers for liver transplants in people with hepatitis C has tripled, from less than 3 percent in 1995 to more than 9 percent in 2013.

    Researchers analyzed data from nearly almost 44,000 people with hepatitis C who received a liver transplant in the United States during that time. Almost 6 percent received an HCV-positive liver. There was no difference in time to death between those who received either a liver with hepatitis or a healthy liver, the study found.

    The study was to be presented Thursday at the International Liver Congress in Barcelona, Spain.

    “Over the past two decades, mainly due to shortages in organs, the use of HCV-positive organs for liver transplantation has tripled,” said study author Dr. Zobair Younossi, chair of the department of medicine at Inova Fairfax Hospital in Falls Church, Va.

    “Our study clearly shows that people with HCV who received HCV-positive livers had the same medium- to long-term outcomes as people that received healthy livers. As highly effective treatments for HCV are available for transplanted patients, the future of these patients is bright,” Younossi added in a Congress news release.

    This study demonstrates a greater opportunity for use of HCV-positive livers due to their comparable outcomes with healthy livers, Tom Hemming Karlsen, vice secretary of the European Association for the Study of the Liver, said in the news release.

    “With the number of people waiting for a liver transplant expected to rise, the study results should give hope over the coming years for those on the waiting list,” he said.

    Data and conclusions presented at meetings are usually considered preliminary until published in a peer-reviewed medical journal.
